The Follow My Child personal health record is a digital medical record that contains foster children’s medical information in one place that you can access anytime, anywhere. The basic components include the general medical record, behavioral health, school information, immunization and medication summary Imagine not having to keep notebooks and piles of papers with your medical records. Imagine not having to carry your information for doctor to doctor. Imagine not having to worry that you might forget an important medication do not remember exactly what medical procedures your foster child had. Now imagine having all your child’s vital health data securely available to you wherever you might be. With Follow My Child you no longer need to imagine....... If you are currently a licensed foster parent in Miami-Dade or Monroe counties, you can have access to your foster child’s personal health record. The health record is titled “Follow My Child” and will provide foster parents and foster children with a summary of their medical and behavioral health.
