The Follow My Child personal health record is a digital medical record with all of your medical information in one place that you can access anytime, anywhere. Follow My Child holds your health records from your primary doctor, your dentist, your obstetrician/gynecologist, and any other providers YOU choose in the general medical record. There is a symptom diary where you can record and date any problems you are having, so when you go to the doctor, you can provide details about your symptoms. Follow My Child has a special permission feature to protect your privacy that lets you decide who is allowed to see what information. Please refer to our policies regarding privacy and permissions. Follow My Child members receive a printable wallet sized emergency card containing the most critical medical and contact information. The administrative tab also contains an Advanced Health Care Directive form. Imagine not having to keep notebooks and piles of paper with your medical records.
