NURSE CASE MANAGER

Nurse Case Managers (NCM) are nurses who work with case workers to provide medical case management for children in foster care. Medical Case Management and care coordination is provided telephonically. Nurse Case Managers ensure that children’s medical needs are met. What role does the Nurse Case Manager play in the care of my child? Among their many responsibilities, nurse case managers help parents understand their child’s current health status and prescribed treatment plan. As patient advocates, they work with other members of your child’s medical team and case worker to ensure that your child is receiving all prescribed care and/or recommendations. Back to top What services are not provided by the Nurse Case Manager? Nurse Case Managers do not provide direct care services. This means, that they are non-clinical and cannot accompany children to doctor’s appointments or visit children in their home. Nurse Case Managers or the Nurse Case Management staff are not Physicians and should not be used as a substitute for Physician Visits or Medical Advice. CALL YOUR PHYSICIAN OR 911 IMMEDIATELY IN THE EVENT OF AN EMERGENCY. Persons providing services through the Program ARE NOT PHYSICIANS. Nurse Case Managers (registered nurses/licensed practical nurses), telephone line attendants and others administering Program services are typically individuals with somewhat limited medical training – unlike highly trained licensed physicians. The Nurse Case Managers are a source of health and wellness information to be used for informational purposes only. Information provided by these individuals is not intended to be a substitute for professional medical advice, diagnosis, or treatment of a physician. Always seek the advice of your child’s physician with any questions you have regarding your child’s overall health or a particular medical condition. Inform your child’s physician of your use of a Nurse Case Manager or other Program resources (including telephone services), if applicable. Never delay seeking professional medical help and never disregard the advice of your physician because of information received from a Nurse Case Manager or any other individual administering or providing services under the Program. Health information is subject to change at any time, and any information obtained as a result of the Program should be confirmed with your child’s physician.
